The attached "Intake form" needs to be printed, completed and returned to Jill by email as an attachment in order to begin the process for consultation. Alternatively, the questions on this form may be cut and pasted directly into an email to jstover@ardassist.com.
While there are several questions, it is important to have the form completed as much as possible in order for Jill to provide helpful and relevant initial recommendations and resources. Some parents will find that the initial response will be sufficient and won't need to go further with services. For others who choose to use ARD Assist for additional services, having a thoroughly completed form will shorten the time Jill will need to spend on the case, and accordingly, fees will be less.
Upon receipt of the completed form and review of the information, Jill will respond to the parent typically via email. The turn around time will generally be within 24-48 hours during regular the M-F time frame, or if the email is received on Friday or the weekend, it may be the next weekday before a response will be provided. |
